The Nursing Clinical Coordinator is responsible for facilitating, planning, advocating, coordinating, and integrating patient care for a designated patient population. Insures the delivery of cost effective, outcome-oriented quality care. Works collaboratively with multidisciplinary members of the healthcare team to
ensure successful progression for the patient through the continuum of care in assigned care area with attention to patient specific needs. Serves as the point person for complex patient issues that may be clinical in nature, service related or due to authorization or coordination of care. Assists in advancing the provision of care through research and educational activities.
- Assists in the creation and/or management of a triage tool to direct patients toward the appropriate specialty care provider.
- Receives triage calls for patient evaluations and refers as appropriate using established triage protocols and provider input as required.
- Works with Division Chief and other practitioners to develop patient education materials and processes.
- Provides pre and postoperative support to assigned specialty area patients and assures continuity of care within various areas in the Network.
- Provides and/or arranges individual or group education for patients.
- Facilitates patient access to appropriate medical and specialty providers as well as other care coordination team support.
- Maintains a clinical schedule for post-operative appointments for patients requiring specialized care.
- Coordinates care of complex patients.
- Ensures effective tracking of test results, medication management, and adherence to follow-up appointments where applicable.
- Acts as an expert resource for assigned specialty area and may be reassigned to clinic during times of staffing shortages, high demand, high acuity, or specialized procedures.
- Assists in the coordination of special equipment/supplies and/or product vendor for procedures as requested.
- Attends department specific huddles and meetings, participates on subcommittees, and partakes in specialty-related conferences.
- Attends Quality and Performance Improvement meetings as assigned.
- Coordinates and facilitates multidisciplinary meetings with ancillary resources, inpatient nursing, PAT, discharge planning and others as appropriate to assess quality of care and review outcomes data and
develop performance improvement plan when applicable. - Conducts assigned quality and performance improvement audits, compares patient outcomes data, and presents information as appropriate.
- Reviews and provides input for modifications of policies and procedures as assigned. May be asked to assist with drafting new policies and procedures.
- Assists in new product review and evaluates for patient safety, efficiency, regulatory compliance, and cost effectiveness.
- Establishes, maintains, and updates patient database and registries as appropriate.
- Performs research and recommends Evidence Based Practice changes.
- Stays apprised to practice standards set forth by relevant professional societies/organizations and brings changes forward for review.
- Assists in training new staff as assigned.
- Provides specialty-related in-services, training, and education to caregivers throughout the network as assigned.
- Participates in departmental research projects.
- May be asked to evaluate patient care and/or work-flow processes and propose improvements within the assigned specialty area.
- Partakes in the development of training and competency validations specific to specialty area as assigned.
- Communicates information needed for the provision of safe, effective and efficient care.
- Ensures patient information is kept confidential and complies with HIPPA standards.
- Communicates with practitioners and care teams to determine their needs and obtain feedback on service provided to their patients.
- Shares quality and patient outcomes information with Departmental Leadership as needed.
- Participates in insurance denials on an as-needed basis.
- Is a resource for insurance approval inquiries.
- Supports schedule utilization and assists in measures to ensure clinic schedules are at full capacity.
- Utilizes resources in an efficient and effective manner.
- Contributes to ensuring effective patient supply and equipment charging processes are in place as requested.
- Performs other duties as requested and observed by supervisor or manager.
Qualifications:
Education:
- 2 Year/Associates Degree in Nursing, required
- 4 Year/Bachelor's Degree in Nursing, preferred
- *** All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN’s licensed on or before 12/19/17 are/remain exempt from this requirement
Experience:
- Minimum 6 months to 1 year experience in Acute Care, required
- 1 year experience in specialty area, Ambulatory or Acute Care, preferred
- 1 year leadership experience, preferred
Licensure/Certifications:
- Current NYS Registered Professional Nurse license, required
- Current BLS certification, required
- Valid NYS Drivers License, required
- additional certifications may be required based on specialty

Physical Requirements:
- Medium Work - Exerting up to 50 lbs. for force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects

Total Rewards:
We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. Bonus Amount:
- RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time
- LPN sign on bonuses are awarded in the amount of $10,000 for full time
